Who we are

What we collect

We only collect what you choose to send us through this website:

  • Care inquiries. Your name, phone number, email address, general location, who needs care, your preferred timeframe, and anything you write in the message box.
  • Employment applications. The information on our online application: the position you want, your contact details, education and credentials, work history, references, emergency contacts, and your electronic signature.

We do not ask for Social Security numbers, dates of birth, driver's license numbers or bank details anywhere on this website. Those details are collected in person, on paper, only after a job offer is made. Please do not send them to us through this site.

We also ask that you not include medical details, diagnoses or health history in website forms. We'll discuss your loved one's care needs by phone or in person instead.

How we use it

  • To respond to your inquiry and discuss whether our services fit your family's needs.
  • To evaluate employment applications, contact your references, and carry out the background and registry checks required by Texas law before hiring anyone with client contact.
  • To keep required personnel and business records, including records we must be able to produce during a Texas HHSC survey or audit.

We do not sell your information. We do not share it with advertisers, and this site does not carry advertising or social-media tracking pixels.

Who can see it

Submissions go to our own business email and are accessible only to Ur Unique Home Health staff involved in intake or hiring. We use service providers to host this website and deliver email; they process data on our behalf and are not permitted to use it for their own purposes.

How long we keep it

Care inquiries are kept only as long as needed to respond and, if you become a client, as part of your client record. Employment applications are retained in accordance with state and federal recordkeeping requirements, including the personnel-record requirements that apply to licensed home and community support services agencies in Texas.

Security

This website is served over an encrypted connection (HTTPS), and form submissions are transmitted encrypted. No system is perfectly secure, which is another reason we deliberately keep sensitive identifiers off this website entirely. If a breach affecting personal information were to occur, we would notify affected individuals and the Texas Attorney General as required by Texas law.

Your choices

  • You can ask us what information we hold about you, ask us to correct it, or ask us to delete it, by emailing admin@uruniquehh.org. Some records we are legally required to keep.
  • You can withdraw an application at any time by contacting us.
  • You can call us at (214) 791-3082 instead of using any form on this site.

Children

This website is not directed to children, and we do not knowingly collect information from anyone under 18. Our employment application is open only to applicants who are 18 or older.
